目的 探讨吴茱萸穴位贴敷结合中药熏洗坐浴对肛周脓肿手术患者术后疼痛程度及创面愈合的影响。方法 选取2024年7月至2024年12月我院115例肛周脓肿手术患者相关资料进行回顾性分析,依据患者治疗方案分为对照组（n=57）和观察组（病n=58）。对照组接受1∶5 000高锰酸钾溶液坐浴,观察组接受吴茱萸穴位贴敷结合中药熏洗坐浴。对比两组术后创面分泌物、创面肉芽组织生长情况、疼痛程度[疼痛数字评分法（NRS）]、肛门功能（肛门失禁Wexner评分系统）、血清炎症因子[降钙素原（procalcitonin,PCT）、白细胞介素-6（interleukin-6,IL-6）]水平、创面愈合情况。结果 与术后1 d相比,两组术后7 d创面分泌物评分、创面肉芽生长评分均降低且观察组较对照组更低（P<0.05）。与术后1 d相比,两组术后7 d疼痛NRS评分、Wexner评分均降低,且观察组较对照组更低。与术后1 d相比,两组术后7 d血清PCT、IL-6水平均下降,且观察组较对照组更低。观察组创面水肿消失时间和创面愈合时间均短于对照组。结论 吴茱萸穴位贴敷结合中药熏洗坐浴可以更好地缓解肛周脓肿手术患者术后疼痛,促进肉芽组织生长,减轻炎症反应,促进创面愈合及肛门功能恢复。

Objective To investigate the influence of combination of Evodia rutaecarpa acupoint application and traditional Chinese medicine fumigation sitz bath on postoperative pain and wound healing in patients receiving perianal abscess surgery. Methods A retrospective analysis was conducted on the relevant data of 115 patients undergoing perianal abscess surgery in our hospital from July 2024 to December 2024. According to the treatment plans of the patients, they were divided into the control group (57 cases) and the observation group (58 cases). The control group received sitz baths with 1∶5000 potassium permanganate solution, while the observation group received sitz baths with Evodia rutaecarpa acupoint application combined with traditional Chinese medicine fumigation and washing. The postoperative wound secretion, wound granulation growth status, pain degree [numerical rating scale (NRS)], anal function (anal incontinence Wexner score system), serum inflammatory factors [procalcitonin (PCT), interleukin-6 (IL-6)] and wound healing status were compared between both groups. Results Compared with 1 day after surgery, the wound secretion score and wound granulation growth score in both groups were decreased at 7 days after surgery, and the scores in the observation group were lower (P<0.05). The pain NRS score and Wexner score in the two groups at 7 days after surgery were reduced compared with those before surgery, and the scores were lower in the observation group. The levels of serum PCT and IL-6 were declined in both groups at 7 days after surgery compared to 1 day after surgery, and the observation group had lower levels. The disappearance time of wound edema and wound healing time were shorter in the observation group than those in the control group. Conclusion Evodia rutaecarpa acupoint application combined with traditional Chinese medicine fumigation sitz bath can relieve the postoperative pain in patients with perianal abscess surgery, promote the granulation growth, reduce inflammatory response, and promote the anal function recovery.
