目的 评价针刺补气穴联合盆底肌训练对产后盆底功能与性生活质量的影响。方法 选取我院2023年6月至2024年6月104例产妇作为研究对象,依据随机数字表法按1∶1比例分成对照组与观察组,各52例,对照组行盆底肌训练;观察组在此基础上联合针刺补气穴治疗。干预1个月及3个月后随访。比较两组盆底肌电活动、肌力评分、性生活质量及盆底功能障碍相关疾病发生率。结果 治疗1个月后,两组盆底肌平均电压、最大电压及肌力评分均较治疗前升高,观察组改善幅度更大。两组性生活质量指标均提升,其中观察组FSFI评分、性生活频率及持续时间优于对照组。观察组性功能障碍、盆腔器官脱垂及压力性尿失禁发生率明显低于对照组。治疗3个月后,观察组性功能障碍和盆腔器官脱垂发生率均为0,明显低于对照组的15.38%（8/52）和17.31%（9/52）;压力性尿失禁发生率低于对照组;观察组总有效率高于对照组,两组安全性比较无统计学差异。结论 针刺补气穴联合盆底肌训练可在产后早期即改善盆底功能及性生活质量,随治疗时间延长疗效更好,能有效预防性功能障碍、尿失禁及脏器脱垂的发生,具有明显时间依赖性和临床价值。
Abstract
Objective To evaluate the effects of acupuncture at tonifying points combined with pelvic floor muscle training on postpartum pelvic floor function and sexual quality of life. Methods A total of 104 postpartum women treated at our hospital between June 2023 and June 2024 were enrolled. Participants were randomly assigned (1∶1) to a control group (n=52) or an intervention group (n=52). The control group received pelvic floor muscle training alone, whereas the intervention group received additional acupuncture at tonifying points. Follow-up assessments were conducted at 1 and 3 months. Outcomes included pelvic floor electromyography, muscle strength scores, sexual function, and the incidence of pelvic floor disorders. Results After 1 month, both groups showed increased mean and maximal pelvic floor muscle voltage as well as muscle strength scores compared with baseline, with greater improvements in the intervention group. Sexual quality of life improved in both groups, but the intervention group had higher Female Sexual Function Index (FSFI) scores, greater intercourse frequency, and longer duration. The incidence of sexual dysfunction, pelvic organ prolapse, and stress urinary incontinence was significantly lower in the intervention group. At 3 months, the intervention group reported no cases of sexual dysfunction or pelvic organ prolapse, compared with 15.38% and 17.31% in the control group. Stress urinary incontinence was less frequent in the intervention group, but the difference was not significant. The overall effective rate was higher in the intervention group. Mild muscle soreness occurred in 5.77% of the intervention group and 3.85% of the control group, with no significant difference in safety. Conclusion Acupuncture combined with pelvic floor muscle training improves postpartum pelvic floor function and sexual quality of life. Benefits increase over time, with significant reductions in sexual dysfunction, urinary incontinence, and organ prolapse. This combined intervention shows clear time-dependent effects and favorable clinical value.
