摘要

目的: 探究低温等离子射频消融术(low-temperature plasma radiofrequency ablation,TCAR)联合胶原酶注射治疗对神经根型颈椎病(cervical spondylotic radiculopathy,CSR)患者颈椎疼痛和功能的影响。方法: 回顾性分析2021年9月—2023年9月收治的CSR患者的临床资料,根据患者的治疗方法,从进行胶原酶注射治疗的患者中随机抽取46例纳入对照组,从TCAR联合胶原酶注射治疗的患者中随机抽取46例纳入联合组。比较治疗后12个月的临床疗效;比较治疗后疼痛水平[视觉模拟评分(visual analogue scale,VAS)]、颈椎功能(cervical spine function,NDI)、颈椎活动度、血清指标水平[环氧合酶-2(cyclooxygenase-2,COX-2)、神经肽Y(neuropeptide Y,NPY)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]差异。结果: 治疗12个月后,联合组临床疗效的总有效率高于对照组。治疗后6个月、12个月时,两组VAS、NDI评分均较治疗前降低,且同组治疗后12个月时低于治疗后6个月时;治疗后联合组各时间点均低于同时期对照组。治疗后6个月时,两组患者颈椎活动度均升高,且联合组高于对照组,两组COX-2、NPY、IL-6、TNF-α均降低,且联合组低于对照组。结论: 使用TCAR联合胶原酶注射治疗能够显著改善CSR患者颈椎疼痛严重程度,恢复颈椎功能,有助于提高患者临床疗效。

Abstract

Objective To investigate the influence of low-temperature plasma radiofrequency ablation (TCAR) combined with collagenase injection on cervical pain and function in patients with cervical spondylotic radiculopathy (CSR). Methods The clinical data of CSR patients admitted from September 2021 to September 2023 were retrospectively analysed, and according to the treatment method of the patients, 46 cases were randomly selected from those who underwent collagenase injection treatment to be included in the control group, and 46 cases were randomly selected from those who underwent TCAR combined with collagenase injection treatment to be included in the combined group. The clinical efficacy at 12 months after treatment and pain degree [visual analogue scale (VAS)], cervical spine function (NDI), cervical spine activities, serum indicators [cyclooxygenase-2 (COX-2), neuropeptide Y (NPY), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α)] at different time points after treatment were compared between groups. Results After 12 months of treatment, the total effective rate of clinical efficacy in combined group was higher than that in control group. The VAS、NDI score in the two groups at 6 months and 12 months after treatment was reduced compared with that before treatment, and the score in the same group at 12 months after treatment was lower than that at 6 months after treatment, and the score at each time point after treatment was lower in combined group than that in control group. At 6 months after treatment, the cervical spine activities were increased in the two groups, and the indicators in combined group were higher compared to control group. The levels of COX-2, NPY, IL-6 and TNF-α in both groups were decreased, and the above levels were lower in combined group than those in control group. Conclusion The application of TCAR combined with collagenase injection can significantly improve the severity of cervical pain in patients with CSR, restore the cervical spine function, and help to enhance the clinical efficacy of patients.
