目的：探讨复方多粘菌素B软膏与磺胺嘧啶银软膏联合封闭负压引流术（vacuum sealing drainage,VSD）在烧伤中应用价值。方法：选取2020年6月—2022年6月我院122例烧伤患者,随机数字表法分为两组,各61例。两组均采用VSD,对照组使用1%磺胺嘧啶银乳膏,研究组使用复方多粘菌素B软膏。观察两组治疗效果、不良反应及治疗前后各项临床症状评分、血管新生因子[血管内皮生长因子（endothelial growth factor,VEGF）、碱性成纤维细胞生长因子（basic fibroblast growth factor,bFGF）]水平、皮肤屏障功能。结果：研究组治疗总有效率（88.52%）较对照组（73.77%）高;治疗21 d后研究组创面红肿水疱、分泌物渗出、疼痛程度评分均低于对照组;VEGF、bFGF水平均高于对照组;皮肤角质层含水量高于对照组,经皮水分丢失量低于对照组;研究组创面感染率、瘢痕发生率（0.00%、3.28%）低于对照组（11.47%、13.11%）。结论：复方多粘菌素B软膏联合VSD有助于缓解烧伤患者临床症状,促进血管新生,改善皮肤屏障功能,提高治疗效果,减少不良反应。
Abstract
Objective To explore the application value of compound polymyxin B ointment combined with vacuum sealing drainage (VSD) in burns. Methods A total of 122 burn patients in our hospital from June 2020 to June 2022 were selected and randomly divided into two groups using a random number table method, with 61 patients in each group. Both groups underwent VSD treatment. The control group was treated with 1% sulfadiazine silver cream, while the study group was treated with compound polymyxin B ointment. The therapeutic effects and adverse reactions of the two groups were observed, as well as various clinical symptom scores, vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF), and skin barrier function before and after 21 days of treatment. Results The total effective rate of treatment in the study group was 88.52%, which was higher than that in the control group (73.77%). After 21 days of treatment, the study group exhibited lower levels of redness, swelling, blisters, secretion exudation, and visual analogue scale (VAS) scores compared to the control group. Additionally, the levels of VEGF and bFGF in the study group were higher than those in the control group after 21 days of treatment. Furthermore, the skin stratum corneum moisture content in the study group was higher than that in the control group after 21 days of treatment, while the transepidermal water loss was lower. The wound infection rate and scar formation rate in the study group (0.00% and 3.28%, respectively) were lower than those in the control group (11.47% and 13.11%, respectively). Conclusion Compound polymyxin B ointment helps alleviate clinical symptoms in burn patients, promote angiogenesis, improve skin barrier function, enhance treatment efficacy, and reduce adverse reactions.
