目的：探究内镜窄带成像技术联合靛胭脂染色对幽门螺杆菌（Helicobacter pylori,Hp）相关性胃癌的诊断价值。方法：回顾性分析2020年6月—2022年1月收治的96例Hp相关性胃病患者的临床资料,所有患者均接受普通白光内镜、内镜窄带成像及靛胭脂染色检查。以病理检查结果为金标准,评估上述检查方式对Hp相关性胃癌患者诊断价值及影像质量评分差异。结果：病理结果显示,96例Hp相关性胃病患者中,确诊胃癌者37例,非胃癌者59例。内镜窄带成像及内镜窄带成像联合靛胭脂染色对Hp相关性胃癌患者的诊断准确度高于普通白光内镜,三者的诊断Kappa值分别为0.675、0.869、0.934,内镜窄带成像、窄带成像联合靛胭脂染色诊断与病理结果具有较高的一致性（Kappa>0.75）;内镜窄带成像、窄带成像联合靛胭脂染色的病变轮廓、微血管形态、胃小凹形态评分均高于普通白光内镜,且窄带成像联合靛胭脂染色上述评分高于内镜窄带成像。结论：内镜窄带成像技术联合靛胭脂染色对Hp相关性胃癌的诊断价值较高,且能获取清晰的影像。

Objective To explore the diagnostic value of endoscopic narrow-band imaging technique combined with indigo carmine staining on Helicobacter pylori (Hp)-associated gastric cancer. Methods Clinical data of 96 patients with Hp-associated gastric diseases admitted to the hospital from June 2020 to January 2022 were retrospectively analyzed. All patients underwent white light endoscopy, endoscopic narrow-band imaging and indigo carmine staining, with pathological examination result as the gold standard. The differences in diagnostic value of the above examination methods and the differences in imaging quality scores of the above methods were compared on patients with Hp-associated gastric cancer. Results Pathological result showed that 37 cases of gastric cancer and 59 cases of non-gastric cancer were confirmed among 96 patients with Hp-associated gastric diseases. The diagnostic accuracy of endoscopic narrow-band imaging and endoscopic narrow-band imaging combined with indigo carmine staining in patients with Hp-associated gastric cancer was higher than that of white light endoscop. The diagnostic Kappa values of the three were 0.675, 0.869, and 0.934, endoscopic narrow-band imaging, endoscopic narrow-band imaging combined with indigo carmine staining showed high consistency with pathological results (Kappa>0.75). The scores of lesion contour, microvascular morphology and gastric pit morphology of endoscopic narrow-band imaging and endoscopic narrow-band imaging combined with indigo carmine staining were higher than those of white light endoscopy, and the above scores of endoscopic narrow-band imaging combined with indigo carmine staining were higher than those of endoscopic narrow-band imaging. Conclusion Endoscopic narrow-band imaging technique combined with indigo carmine staining is valuable in the diagnosis of Hp-associated gastric cancer and can obtain clear images.
