目的：评估改良式内镜下泪道重建术（En-DCR）联合贝复舒凝胶吻合口填塞治疗慢性泪囊炎的早期疗效。方法：本研究选取2020年10月—2023年10月期间江苏大学附属人民医院接诊的慢性泪囊炎患者92例。通过随机数字表法分为参照组（n=46）和联合组（n=46）。两组均采用改良式En-DCR术治疗,联合组采用贝复舒凝胶填塞吻合口。比较两组患者的临床疗效、泪道功能和症状评估、术后恢复情况、患者满意度以及生活质量。结果：联合组的总有效率及总满意率高于参照组。治疗后联合组的溢泪程度评分低于参照组,BUT、泪道畅通率高于参照组。治疗后6个月联合组的吻合口面积、肉芽检出率、复发率均低于参照组,生活质量（SF-36评分）高于参照组。结论：改良式En-DCR联合贝复舒凝胶吻合口填塞可显著提高慢性泪囊炎患者的术后泪道通畅率,改善泪液分泌,且安全性好,值得临床推广应用。
Abstract
Objective To assess the initial treatment outcomes of chronic dacryocystitis using modified endoscopic dacryocystorhinostomy (En-DCR) with Befulxu gel. Methods This study enrolled 92 patients with chronic dacryocystitis treated at the People's Hospital affiliated with Jiangsu University from October 2020 to October 2023. Patients were randomly assigned to either the reference group (n=46) or the combined group (n=46) using a random number table method. Both groups underwent modified En-DCR; the combined group additionally received Beifushu gel packing at the anastomotic site. Clinical efficacy, lacrimal duct function and symptom assessment, postoperative recovery, patient satisfaction, and quality of life were compared between the two groups. Results The overall effectiveness and satisfaction rates were higher in the combined group compared to the reference group. Post-treatment, the combined group exhibited lower scores for tear overflow, higher tear film breakup time (BUT), and better patency of the lacrimal duct than the reference group. Six months after treatment, the combined group showed smaller anastomosis site areas, lower granulation tissue detection rates, and lower recurrence rates, along with higher SF-36 scores than the reference group. Conclusion Modified En-DCR combined with Befulxu gel packing significantly improves the patency of the lacrimal duct postoperatively, enhances tear secretion, and is safe, thus meriting broader clinical application.
