目的: 检测成人急性白血病血清甘露糖结合凝集素（Mannose-bindingLectin,MBL）、甘露糖结合凝集素相关丝氨酸蛋白酶-1（mannose-bindinglectinassociatedserineprotease-1,MASP1）和甘露糖结合凝集素相关丝氨酸蛋白酶-2（mannose-bindinglectinassociatedserineprotease-2,MASP2）水平并研究其临床价值。方法: 采用酶联免疫吸附试验（ELISA）检测成人急性白血病患者及正常健康人血清MBL、MASP1和MASP2水平,采用ROC曲线分析血清MBL、MASP1和MASP2在诊断成人急性白血病时的灵敏度、特异度、准确度。结果: 白血病组和对照组研究对象在MBL方面的差异无统计学意义,白血病组患者血清MASP1水平显著低于对照组健康人,而血清MASP2显著高于对照组健康人。复发白血病患者血清MBL水平显著低于初发白血病患者,MASP2显著高于初发白血病患者,而两组间血清MASP1差异无统计学意义。Logistics回归分析表明血清MBL、MASP1、MASP2水平和成人白血病均有一定的关联。MBL、MASP1联合MASP2诊断成人急性白血病灵敏度、特异性、准确度分别为0.854、0.810和0.664。结论: 成人白血病患者血清MASP1显著降低而 MASP2显著升高,MBL、MASP1及MASP2联合检测对成人白血病具有较高的诊断价值。
Abstract
Objective To Detect the serum levels of MBL, MASP1 and MASP2 in adults with acute leukemia and study their clinical value Methods Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of serum MBL, MASP1 and MASP2 in adult acute leukemia patients and normal healthy people, and receiver operating characteristic curve was used to analyze the sensitivity, specificity and accuracy of serum MBL, MASP1 and MASP2 in the diagnosis of adult acute leukemia. Results There was no significant difference in MBL between the leukemia group and the control group. The serum MASP1 level of the leukemia group was significantly lower than that of the control group, while the serum MASP2 level was significantly higher than that of the control group. People. The serum MBL level in patients with relapsed leukemia was significantly lower than that in patients with first-onset leukemia, and the level of MASP2 was significantly higher than that in patients with first-onset leukemia, while there was no significant difference in serum MASP1 between the two groups. Logistic regression analysis showed that serum levels of MBL, MASP1, MASP2 were associated with adult leukemia. The sensitivity, specificity and accuracy of MBL, MASP1 combined with MASP2 in the diagnosis of adult acute leukemia were 0.854, 0.810 and 0.664, respectively. Conclusion Serum MASP1 was significantly decreased while MASP2 was significantly increased in adult leukemia patients. The combined detection of MBL, MASP1 and MASP2 has a high diagnostic value for adult leukemia.
