目的: 调查硅肺患者外周血T细胞表达与肺功能硅状态、生存质量的关系。方法: 选取2019年9月-2021年3月在本院接受硅肺住院治疗的90例患者作为研究对象,纳入硅肺组。同时,选取同期在本院接受体检的90例健康人员作为健康对照组。比较两组外周血T细胞表达的差异,并分析硅肺组患者外周血T细胞表达与肺功功能状态、生存质量的关系。结果: 与健康对照组受试者相比,硅肺组患者具有较低的FEV1、FEV1/FVC、DLco、CD4+ T细胞和CD4+/CD8+,以及较高的RV和CD8+ T细胞。Ⅰ期、Ⅱ期和Ⅲ期硅肺组患者在FEV1、FEV1/FVC、RV和SGRQ的症状评分、活动评分和总分存在显著差异。CD8+ T细胞的百分比与DLco呈负相关,和与症状评分和活动评分呈正相关。CD4+ T细胞的百分比与FEV1和FEV1/FVC呈负相关。结论: 硅肺患者外周血中CD8+ T细胞百分比与硅肺患者的气体交换异常程度正相关,而CD4+ T细胞百分比与气体交换异常程度负相关。
Abstract
Objective To investigate the relationship between peripheral blood T-cell Profiles and pulmonary function and quality of life in silicosis patients. Methods 90 patients who underwent silicosis hospitalization in our hospital from September 2019-March 2021 were enrolled in the silicosis group. At the same time, 90 healthy people who underwent physical examination at the same time were selected as healthy controls. The differences of peripheral blood T cell expression between the two groups were compared, and the relationship between peripheral blood T cell expression and lung function status and quality of life in silicosis group was analyzed. Results Compared with control subjects, patients with silicosis had lower FEV1, FEV1/FVC, DLco, CD4+T cells and CD4+/CD8+, and higher RV and CD8+T cells. There were significant differences in FEV1, FEV1/FVC, RV and SGRQ symptom score, activity score and total score among stageⅠ, Ⅱ and Ⅲ silicosis patients. The percentage of CD8+ T cells was negatively correlated with DLco, and positively correlated with symptom score and activity score. The percentage of CD4+ T cells was negatively correlated with FEV1 and FEV1/FVC. Conclusion The percentage of CD8+ T cells in peripheral blood of silicosis patients is positively correlated with the abnormal degree of gas exchange, while the percentage of CD4+ T cells is negatively correlated with the abnormal degree of gas exchange.
