Efficacy of ginkgo leaf extract dipyridamole injection combined with alprostadil in the treatment of patients with vertebrobasilar insufficiency vertigo and its influence on hemodynamics

Abstract:Objective To mainly explore the influence of ginkgo leaf extract dipyridamole injection and alprostadil on patients with vertebrobasilar insufficiency vertigo (VBIV). Methods 150 patients with VBIV were selected from May 2019 to April 2023, and they were divided into control group and observation group by the odd-even number method, with 75 cases in each group. Control group was treated with alprostadil (PGE1), and observation group was given PGE1 combined with ginkgo leaf extract dipyridamole. The clinical efficacy after 2 weeks of treatment, hemodynamics [left vertebral artery (LVA) flow velocity, right vertebral artery (RVA) flow velocity, basal artery flow velocity (BA) ], hemorheology [erythrocyte sedimentation rate (ESR), hematocrit (HCT), whole blood high-shear viscosity (HSV), whole blood low-shear viscosity (LSV) ], oxidative stress indicators [serum superoxide dismutase (SOD), nitric oxide (NO), malondialdehyde (MDA), glutathione peroxidase (GSH-Px) ], inflammatory factors [hypersensitive C-reactive protein (hs-CRP), interleukin-6(IL-6) ] and vertigo degree[Visual Vertigo Analogue Scale (VVAS), dizziness handicap inventory (DHI) ] before treatment and after 2 weeks of treatment and drug safety during treatment were compared between both groups. Results After 2 weeks of treatment, the total effective rate of efficacy in observation group was significantly higher compared with that in control group. The LVA, RVA, BA, SOD, NO and GSH-Px in both groups after 2 weeks of treatment were enhanced compared with those before treatment, and the indicators were higher in observation group. The ESR, HCT, HSV, LSV, MDA, hs-CRP and IL-6 were reduced after 2 weeks of treatment in the two groups, and the indicators in observation group were lower in comparison with control group. After 2 weeks of treatment, the scores of VVAS and DHI in both groups were significantly decreased compared with those before treatment, and the scores in observation group were lower compared to control group. During treatment, the differences were not significant in the incidence rates of adverse reactions between the two groups. Conclusion Ginkgo leaf extract dipyridamole injection combined with alprostadil has a significant effect in treating VBIV, and it can effectively restore the hemodynamics and hemorheology, relieve the inflammatory response and oxidative stress response, and help improve vertigo symptoms, with high safety.
