Diagnostic Study of Spiral CT and Three-Dimensional Reconstruction Technique in Evans Typing and Stability of Intertrochanteric Fracture of Femur

Abstract

Objective To explore the diagnostic value of spiral CT and three-dimensional reconstruction technique in Evans typing and stability of intertrochanteric fracture of femur (IFF). Methods A retrospective analysis was conducted on the case data of 133 IFF patients treated in our hospital from January 2021 to February 2025. All patients underwent X-ray examination and spiral CT examination. The diagnostic value of the two examination methods for Evans classification and stability was compared. Results Intraoperative diagnosis of Evans typing in all patients showed that there were 47 cases of type I, 8 cases of type II, 35 cases of type III, 20 cases of type IV and 23 cases of type V. Compared with intraoperative diagnosis, X-ray diagnosis had certain diagnostic efficiency on Evans typing of IFF patients. Spiral CT combined with three-dimensional reconstruction technique had high diagnostic efficiency on Evans typing, and the accuracy rates of different types were 94.74%, 90.98%, 81.95%, 81.20% and 90.23% respectively. However, for type II and type IV patients, the consistency with intraoperative diagnosis was the lowest among the 5 types (Kappa=0.353, 0.379). The accuracy of the two diagnostic methods was significantly different. The diagnostic accuracy rates for external wall integrity, coronal fracture and fracture end stability were higher than those of X-ray (P<0.05). Conclusion IFF patients with different Evans types have different CT signs. Spiral CT and three-dimensional reconstruction technique have high diagnostic value on Evans typing and stability of IFF patients.
