Objective To investigate the current status of clinical decision-making awareness among nursing interns, analyze their latent profile characteristics and influencing factors, and provide theoretical basis for developing targeted educational interventions. Methods By using convenience sampling method, selected nursing interns from secondary and tertiary hospitals in Hunan, Guangdong, Guizhou, Hainan, Hubei provinces, Guangxi Zhuang Autonomous Region, as well as Beijing and Chongqing municipalities between September 2023 and August 2024 as research participants. The general information questionnaires the Clinical Decision-Making in Nursing Scale (CDMNS), and the Clinical Competency Self-Evaluation Scale were employed as research instruments. The 40 items from CDMNS served as explicit indicators for the latent profile analysis. Logistic regression analysis was then conducted to explore the influencing factors of different latent profiles. Results A total of 1 322 nursing interns were ultimately included, with an average item score of (127.91±31.65) on CDMNS. The clinical decision-making awareness of nursing interns could be classified into three latent profiles: the Low Decision-Making-Intuitive Interpretation Group (614/1, 322, 46.44%), the Moderate Decision-Making-Systematic Analysis Group (439/1, 322, 33.21%), and the High Decision-Making-Rational Coping Group (269/1, 322, 20.35%). Logistic regression analysis revealed that the level of the internship hospital, educational background, internship stage, willingness to pursue a nursing career, and self-evaluated clinical competence were influencing factors of nursing interns' clinical decision-making awareness (P<0.05). Conclusion Nursing interns demonstrate significant heterogeneity in clinical decision-making awareness. Nursing educators should implement personalized theoretical instruction, standardized clinical practice, and flexible decision-making training tailored to interns with different characteristics to enhance their clinical decision-making awareness.
