Explore the correlation between serum procollagen type Ⅲ N-terminal propeptide levels and senile sarcopenia

Abstract

Objective To explore the relationship between serum procollagen type Ⅲ N-terminal propeptide (P3NP) and sarcopenia in the elderly, and to evaluate its predictive value. Methods A retrospective analysis was conducted on 130 elderly patients admitted to the Geriatric Department of Xuanwu Hospital, Capital Medical University from 2022 to 2024. According to the consensus of the Asian Working Group for Sarcopenia (AWGS), 36 cases of male sarcopenia, 29 cases of non-sarcopenia patients, 27 cases of female sarcopenia, and 38 cases of female non-sarcopenia were enrolled. Stratified by gender, the clinical characteristics and P3NP levels of the two groups were compared. Spearman correlation analysis was used to explore the correlation between P3NP and muscle strength and physical function. Multivariate Logistic regression analysis was used to analyze the risk factors of sarcopenia in the elderly. ROC curve was used to analyze the predictive efficacy of serum P3NP levels for sarcopenia in the elderly. Results The P3NP level in the sarcopenia group was significantly higher than that in the non-sarcopenia group (P<0.001). The P3NP level in female was significantly negatively correlated with grip strength, 6-meter walking speed, and the times of rises per 10 seconds (P<0.05). Multivariate Logistic regression analysis showed that P3NP (male OR=1.629, 95%CI: 1.243-2.133, P<0.001; female OR=1.391, 95%CI: 1.083-1.786, P=0.010) was an independent risk factor for sarcopenia. ROC curve showed that the area under the curve for predicting sarcopenia was 0.83(male) and 0.81(female). Conclusions The elevated P3NP level is associated with sarcopenia in the elderly, and it has good predictive value for sarcopenia. However, further research is still needed.
