Clinical evaluation of tympanoplasty based on tragus cartilage perichondrium complex micro flap explantation in continuous perfusion mode otoendoscopy

Abstract

Objective To evaluate the clinical efficacy of tympanoplasty for repairing tympanic membrane perforation using a microflap exoplantation technique auricle tragal cartilage-perichondrium composite during continuous perfusion mode otoscopic surgery. Methods A total of 100 patients (102 ears) undergoing tympanoplasty admitted to the Otorhinolaryngology Department of Jianyang People's Hospital between January 2021 and January 2024 were selected as study subjects. Using simple randomization, patients were divided into an observation group (microflap exoplantation technique, n=50) and a control group (conventional skin flap exoplantation technique, n=50). Both groups were repaired with tragal cartilage-perichondrium composite and underwent surgery via otoscopy. Postoperative follow-up assessments were conducted on operation duration, intraoperative blood loss, tympanic membrane healing rate, and hearing recovery. Results The observation group showed significantly better outcomes in terms of operation duration, intraoperative blood loss, and hospital stay compared to the control group. Both groups exhibited significant improvements in hearing indices at 6 months postoperatively compared to preoperative levels, but there were no significant differences between the two groups. The tympanic membrane healing rates in the observation group at 1 and 6 months postoperatively were 98.04% and 100.00%, respectively, with no statistically significant difference compared to the control group. Conclusion Continuous perfusion mode otoscopic surgery combined with the microflap exoplantation technique using tragal cartilage-perichondrium composite is an efficient and safe tympanic membrane repair technique that offers advantages such as shortened operation duration, reduced blood loss, and improved postoperative recovery quality, making it suitable for widespread clinical application.
