The effect of individualized precise sinus hormone administration combined with functional endoscopic sinus surgery on chronic sinusitis and its impact on inflammatory factors and eosinophil counts

Abstract

Objective To explore the clinical value of individualized precise sinus hormone administration combined with functional endoscopic sinus surgery (FESS) in the treatment of chronic rhinosinusitis (CRS). Methods A total of 99 patients with CRS from January 2022 to January 2023 were randomly divided into two groups (49 cases in the control group and 50 cases in the study group). Both groups received conventional comprehensive treatment. On this basis, the control group received FESS treatment, while the study group received individualized precision nasal sinus hormone administration combined with FESS treatment. Compare the perioperative indexes and surgical efficacy, as well as inflammatory factors [interleukin (IL-8, IL-17), tumor necrosis factor-α (TNF-α)], nasal mucociliary function (ciliary clearance rate, ciliary clearance rate), positive number of eosinophils (EOS), positive rate of vascular endothelial growth factor (VEGF), cyclooxygenase-2 (COX-2), nasal mucociliary function (ciliary clearance rate, ciliary clearance speed), and symptom improvement (Lund-Kennedy score, VAS score, SNOT-20 score) before and after surgery between the two groups. Results The study group had a shorter time to restore nasal ventilation and hospitalization time than the control group, and the overall surgical efficacy was higher than the control group (P<0.05). The levels of serum IL-8, IL-17, and TNF-α in the study group were lower than those in the control group on postoperative day 3 and day 7, and the number of peripheral blood EOS and the positive rates of VEGF and COX-2 were lower than those in the control group (P<0.05). The ciliary clearance rate and ciliary clearance speed in the study group were higher than those in the control group at 1 month and 3 months after surgery, while the Lund-Kennedy, VAS, and SNOT-20 scores were lower than those in the control group (P<0.05). Conclusion The individualized precise nasal sinus hormone administration combined with FESS has significant therapeutic effects in the treatment of CRS, reducing the degree of inflammatory response, restoring the function of nasal mucociliary clearance, promoting disease recovery, alleviating symptoms, and improving the quality of life of patients.
