Abstract

Objective This study aims to deeply analyze the cost-effectiveness of the free cervical and breast cancer (collectively referred to as “two cancers”)screening program for women aged 35-64 in Changsha City from 2018 to 2023, providing a basis for scientifically conducting two-cancer prevention and control and for health administrative departments to formulate optimal screening strategies. Method We evaluated the detection rates of cervical and breast cancer, early diagnosis rates, average costs for detecting early-stage cases, and early detection cost indices by analyzing the data from the Changsha City Women's Two-Cancer Free Screening Management System. Results From 2018 to 2023, Changsha City provided free “two cancers”screening services to over one million women aged 35-64. The detection rate for cervical cancer was 46.45 per 100 000, with an early diagnosis rate of 95.64%. The average cost for detecting early-stage cases was 19 927 yuan, and the Early Detection Cost Index (EDCI) was 0.15. The detection rate for breast cancer was 90.87 per 100 000, with an early diagnosis rate of 74.92%. The average cost for detecting early-stage cases was 117 514 yuan, and the EDCI was 0.87. These data indicate that the two-cancer screening program has significant cost-effectiveness value. Conclusion The “two cancers” screening program for women in Changsha City from 2018 to 2023 has achieved remarkable success and demonstrates high cost-effectiveness value. Therefore, it is worthwhile for health, finance, women's federation, and other departments to continue promoting and applying this program collaboratively.
