目的：研究分析Er：YAG激光联合牙周基础治疗慢性牙周炎的临床疗效及对患牙龈沟液中细胞因子的影响。方法：选取2016年8月~2017年7月期间在石家庄市第二医院接受诊治的120例慢性牙周炎患者作为研究对象，根据随机数表法分为传统药物组、激光组，各60例。传统药物组患者接受常规的牙周基础治疗，激光组患者接受Er：YAG激光联合牙周基础治疗。比较两组患者治疗前后牙周临床指标、龈沟液细胞因子水平变化。结果：治疗前，两组患者的牙周临床指标差异无统计学意义；治疗后两组患者的牙周临床指标（BI、PD、CAL）水平均低于治疗前，且激光组低于传统药物组，差异有统计学意义。两组治疗前的龈沟液细胞因子（IL-6、IL-8、TNF-α）指标差异无统计学意义；治疗后两组患者的IL-6、IL-8、TNF-α均低于治疗前，且激光组低于传统药物组，差异有统计学意义。结论：Er：YAG激光联合牙周基础治疗慢性牙周炎的临床疗效显著，能有效减轻患者的患牙牙周炎症反应，改善患者的牙龈出血症状，值得临床推广。

 Objective To study the clinical efficacy of Er: YAG laser in combination with periodontal treatment of chronic periodontitis and its effect on cytokines in gingival crevicular fluid. Methods 120 cases of chronic periodontitis patients admitted to the Second Hospital of Shijiazhuang City from August 2016 to July 2017 were selected as the subjects of this study. All the patients according to random number table method is divided into traditional medicine group and laser group, each group of 60 cases. The patients in the Traditional medicine group were given conventional periodontal basis of treatment, and the patients in the Laser group patients were given Er: YAG laser combined with periodontal for treatment. The clinical indicators of periodontal before and after treatment and the changes of cytokine levels in gingival crevicular fluid in both groups were observed and compared. Results Before the treatment, there was no significant difference in periodontal clinical parameters between the two groups. After treatment, the levels of periodontal clinical parameters (BI, PD, CAL) in both groups were lower than those before treatment, and the laser group was lower than the traditional group. The difference was statistically significant. The levels of cytokines (IL-6, IL-8, TNF-α) in gingival crevicular fluid before treatment in both groups showed no significant difference. After treatment, the levels of IL-6, IL-8 and TNF-α in both groups were lower than those before treatment, and the difference between the two groups was statistically significant. Conclusion Er: YAG laser combined with periodontal treatment of chronic periodontitis significant clinical efficacy, can effectively reduce the patient’s periodontal inflammatory response to improve the patient’s gingival bleeding, deserved clinical promotion.
